HERMeS is a project coordinated by the Bellvitge University Hospital and the Bellvitge Biomedical Research Institute (IDIBELL) in collaboration with i2Cat and Vodafone. Its purpose is to evaluate the effectiveness of monitoring patients with heart failure in a context of multidisciplinary and integrated care between the hospital and community settings through daily telemonitoring of signs, symptoms and structured remote monitoring through videoconference. For remote monitoring, the PIRENe platform is used, jointly developed in its clinical part by IDIBELL and i2Cat, through the integration of connected devices, including the LifeVit BL-2000 smart scale.and a blood pressure monitor, to obtain biometrics that allow preventive care for relapses of the disease. This project includes heart failure care centers spread throughout the Spanish geography and coordinated by the Bellvitge University Hospital and IDIBELL.
